Nurse practitioner provides medical care on campus. Health services provided by the nurse practitioner must be satisfied by insurance or some other method of payment.

Medical Forms
Medical Form: All students are required to complete Ï㽶Ðã’s required Medical Form. This form includes proof of required vaccinations and TB screening or required skin test results.
Religious Immunization Waiver: Students may request a waiver from these immunization requirements on the basis of a sincerely held religious belief. Students requesting such a waiver must review and execute this form and provide it with other documents required for enrollment.
